Initial inquiry information
The Navigation Review form collects your name, country of residence, email, preferred contact method, transplant category, consent choices, and any optional contact, age, destination, hospital, or brief case-overview information you choose to provide. Source-page and campaign details may also be recorded so MedTransplant can understand how the request reached us.
Why this information is collected
MedTransplant uses this information to understand the initial request, prepare an appropriate response, coordinate next steps, and protect the form from misuse. Submitting the form authorizes MedTransplant to contact you about the inquiry using your selected method.
Optional medical documents
You may optionally upload available medical reports, test results, or diagnostic images with the initial inquiry to support case coordination and specialist review. Document consent is requested separately when files are selected. The inquiry is registered before upload, so a file failure does not remove the request.
Documents are stored in private storage and are not made available through public URLs. Access is intended to be limited to authorized operational workflows. MedTransplant does not claim unverified regulatory certification or end-to-end encryption.
Important limitation
An initial inquiry is not a medical diagnosis, confirmation of treatment, hospital acceptance, donor approval, allocation decision, or waiting-list access. Licensed transplant hospitals make all clinical and treatment decisions.
Service delivery and security
Hosting, form-security, database, private-storage, and transactional-email providers process information needed to deliver, protect, store, and acknowledge the inquiry. MedTransplant does not make unsupported compliance, certification, or end-to-end encryption claims.
